CORC User Registration

To access CORC, all hospitals must first complete the following forms:

• User Account Administrator (UAA) Agreement and instructions

CORC Data Reporting

• 2011 CABG Data Abstract Reporting Form
  Abstraction Form for CABG surgeries on or after July 1, 2011
• 2008 CABG Data Abstract Reporting Form
  Abstraction Form for CABG surgeries on or after January 1, 2008 - June 30, 2011
• Surgeon Certification Form
